Панические атаки и паническое расстройство

Пожалуй, большинство из вас слышали о панических атаках. Однако далеко не все понимают, насколько ужасным для человека может быть переживание таких атак, почему они возникают и как помочь себе или близкому, страдающему от них.

В первую очередь, важно понять разницу между паническими атаками и тревогой. Тревога связана с переживаниями относительно того плохого, что может случиться, в то время как паническая атака – это ощущение, что что-то ужасное происходит в этот самый момент. Во время панической атаки человек испытывает не просто сильный страх или тревогу, а взрыв ужаса.

Панические атаки довольно распространены. По статистике, около 23% людей переживают хотя бы один эпизод панической атаки в своей жизни. Однако многие зачастую и не подозревают, что происходящее с ними на самом деле является панической атакой, и долгое время могут полагать, что у них серьезные проблемы со здоровьем.

СИМПТОМЫ

Во время панической атаки у человека, как правило, резко учащается сердцебиение, его бросает в пот, холод или жар, может появиться дрожь, тошнота, ощущение, что не хватает воздуха и трудно дышать, человек может испытывать головокружение, боль или дискомфорт в груди, ощущение нереальности и «выхода» из своего тела, а также страх потери контроля, страх сойти с ума, а нередко – и парализующий страх смерти.

ОПАСНО ЛИ ЭТО?

Сами по себе панические атаки считаются безопасными для здоровья. То есть вы вряд ли действительно сойдете с ума или умрете. Однако, учитывая список вышеописанных физических ощущений, неудивительно, что многие считают происходящее с ними чем-то опасным.

РИСКИ

В связи с этим по прошествии панического эпизода человек может начать постоянно переживать, что подобное может повториться, бояться, что ему что-то угрожает. То есть может появиться страх самой панической атаки. В результате человек начнет избегать ситуаций, которые могут спровоцировать атаку. Если такое происходит, то можно сказать, что развивается так называемое паническое расстройство.

Это может значительно ухудшить качество жизни человека, переживающего расстройство, так как он может перестать заниматься чем-то интересным, приятным и способствующим его развитию (например, путешествовать, ходить в театр, спортзал, общаться с незнакомыми людьми и пр.). Более того, хроническое нахождение в стрессовом состоянии действительно может привести к развитию сердечно-сосудистых и других серьезных заболеваний.

ПСИХОФИЗИОЛОГИЯ

Паническая атака происходит в результате активации врожденной инстинктивной системы самозащиты организма, известной как «дерись – беги – замри». Она молниеносно включается при встрече с неотвратимой угрозой для жизни (например, при столкновении с хищником в джунглях). При панической атаке эта система не просто активируется, как, например, при тревоге или злости, а активируется очень резко и до максимального уровня. Симптомы атаки – это, по сути, проявления активации этой системы защиты. Организм готовит тело к использованию мышц: сердце качает кровь, легкие насыщаются кислородом и т. п.

Но современный человек практически никогда не сталкивается с дикими животными. Так почему же эта экстренная система защиты срабатывает так часто?

ПСИХОЛОГИЧЕСКИЕ ПРИЧИНЫ

Катастрофические интерпретации

Большинство психологов придерживается следующего мнения: паническая атака начинается с того, что мы испытываем какие-то ощущения в нашем теле и ошибочно «катастрофически» их интерпретируем.

У нас где-то кольнуло? Или немного быстрее забилось сердце? Или закружилась голова? Мы воспринимаем это как знак: с нами что-то не так – мы умираем, сходим с ума, теряем сознание и т. п. В виде реакции на эту картину воображаемой катастрофы у нас резко и до предела запускается экстренная система защиты.

Схема уязвимости

Еще одной причиной панических атак является так называемая «схема уязвимости», то есть глубинное убеждение в том, что мир не безопасен. Это выражается в практически не проходящем фоновом ощущении беспокойства и тревоги: вот-вот случится что-то плохое, а мы не готовы с этим справиться.

Схема уязвимости делает таких людей подверженными не только паническим атакам, но и тревожности, и негативным эмоциям в целом. Многочисленные исследования показали, что, как правило, панические атаки – и даже в большей степени паническое расстройство – возникают у людей, которые страдают от хронической тревожности, фобий, навязчивых идей, депрессии и другого рода негативных реакций.

Важно понимать, что некоторые заболевания (гормональные нарушения, астма, аритмия и пр.) могут повышать склонность к паническим атакам. Однако если человек уверен в себе и не склонен к катастрофическим интерпретациям, то сами по себе эти заболевания не могут стать причиной атак.

КАК СПРАВЛЯТЬСЯ

● Научитесь распознавать признаки возможной атаки в своем теле (учащение сердцебиения, повышение потливости и т. п.) и начинайте отключать их, вместо того чтобы еще больше активировать «сигнал тревоги». Самым действенным здесь является глубокое и медленное дыхание: четыре секунды – медленный вдох, задержка дыхания на четыре секунды – и медленный четырехсекундный выдох. Такое дыхание посылает сигнал мозгу, что вы в безопасности, и вам не нужен кислород, чтобы убегать или драться.

● Параллельно с дыханием проговаривайте себе (или тому, с кем это происходит), что все хорошо, это всего лишь тревога, это нормально, ничего страшного не происходит, что это просто мысли, а не реальность, что это пройдет и т. п. Важно, чтобы слова произносились приятным спокойным и полным любви голосом, а не звучали как критика.

● Не избегайте ситуаций, которые вызывают у вас тревогу, а наоборот, погружайтесь в них. Поверьте, с каждым разом вам будет все менее и менее страшно. Этот процесс называется «десенсибилизация» – снижение чувствительности, в данном случае к кажущейся опасной ситуации. Уже многие годы этот метод считается самым эффективным при избавлении от фобий.

● Если вы, в принципе, тревожный человек и живете в постоянном ожидании катастрофы, с которой вы, по вашим ощущениям, не сможете справиться, постарайтесь избавиться от этого восприятия. Важно понять корни этой схемы, залечить старые эмоциональные раны и научиться действовать с позиции уверенного в себе и сбалансированного взрослого. Нужно настроиться на то, что это долгосрочный проект, работать над которым вы можете сами или при помощи специалистов.

    1            Application of terms

    1.1        These Terms apply to your use of the Website. By accessing and using the Website:

    a            you agree to these Terms; and

    b            where your access and use is on behalf of another person (e.g. a company), you confirm that you are authorised to, and do in fact, agree to these Terms

    on that person’s behalf and that, by agreeing to these Terms on that person’s behalf, that person is bound by these Terms.

    1.2        If you do not agree to these Terms, you are not authorised to access and use the Website, and you must immediately stop doing so.

    3            Definitions

    In these Terms:

    Loss includes loss of profits, savings, revenue or data, and any other claim, damage, loss, liability and cost, including legal costs on a solicitor and own client basis

    personal information means information about an identifiable, living person

    Terms means these terms and conditions titled Terms and conditions of use

    Underlying System means any network, system, software, data or material that underlies or is connected to the Website

    You means you or, if clause 1.1b applies, both you and the other person on whose behalf you are acting.

     

    4            Your obligations

    4.1        You must provide true, current and complete information in your dealings with us and must promptly update that information as required so that the

    information remains true, current and complete.

    4.2        You must:

    a not act in a way, or use or introduce anything (including any virus, worm, Trojan horse, timebomb, keystroke logger, spyware or other similar feature) that in any way compromises, or may compromise, the Website or any Underlying System, or otherwise attempt to damage or interfere with the Website or any Underlying System; and

    b unless with our agreement, access the Website via standard web browsers only and not by any other method. Other methods include scraping, deep-linking, harvesting, data mining, use of a robot or spider, automation, or any similar data gathering, extraction or monitoring method.

    10.2      These Terms, and any dispute relating to these Terms or the Website, are governed by and must be interpreted in accordance with the laws of the

    Republic of Cyprus.

    Each party submits to the non-exclusive jurisdiction of the Courts of the Republic of Cyprus in relation to any dispute connected with these Terms or the

    Website.

    10.3      For us to waive a right under these Terms, the waiver must be in writing.

    10.4      Clauses which, by their nature, are intended to survive termination of these Terms, including clauses 5, 6, 7, 10.1, continue in force.

    10.5      If any part or provision of these Terms is or becomes illegal, unenforceable, or invalid, that part or provision is deemed to be modified to the extent

    required to remedy the illegality, unenforceability or invalidity. If a modification is not possible, the part or provision must be treated for all purposes as

    severed from these Terms. The remainder of these Terms will be binding on you.

    10.6      These Terms set out everything agreed by the parties relating to your use of the Website and supersede and cancel anything discussed, exchanged or

    agreed prior to you agreeing to these Terms. The parties have not relied on any representation, warranty or agreement relating to the Website that is not

    expressly set out in the Terms, and no such representation, warranty or agreement has any effect from the date you agreed to these Terms.
